How to Refer Your Patient to New York Center for Living

The following outline details how we here at New York Center for Living plan to collaborate with you during the patient referral process.

If a patient at your practice is deemed clinically appropriate by the staff (substance use primary and between the ages of 13 and 30, displaying a willingness to remain abstinent), your team should directly connect with New York Center for Living to begin the referral process for a seamless transition for the patient’s aftercare. This connection is ideally made at least a week in advance of the patient’s discharge.

Your staff will have the patient sign a consent to release information and send the following clinical information to us:

  • Bio-psycho-social
  • History and physical
  • Current list of medications
  • Discharge paperwork, if already prepared

New York Center for Living will also gather the contact information for the patient’s parents or guardians and/or insurance policy holder/party responsible for payment.

Patient Assessment

If possible, New York Center for Living will interview the patient over the phone to ensure that they display the necessary criteria to receive our level of care. After being deemed appropriate by New York Center for Living, an assessment will be scheduled. All treatment team members, i.e. discharging clinicians, points of contact, family members and patient themselves, will be made aware of the time, place and location of the patient’s assessment with New York Center for Living. We can help facilitate transportation and provide any other insight for additional support that is needed in NYC for the discharging patient’s aftercare plan.

New York Center for Living will contact your team to report if the client’s assessment was completed successfully or if a reschedule was needed or a no-show occurred.

Your team will be contacted once the clinical team has met and determined the patient’s appropriateness for admission into our program. In the case that, during the formal assessment portion of the referral process, the patient is found to be inappropriate for our level of care, New York Center for Living will immediately contact you to effectively collaborate next steps for the patient.

Once accepted, weekly updates regarding the patient’s progress will be provided to your team while the patient is in treatment at New York Center for Living.
